Health Professions Questionnaire

We in Health Professions Advising regard ourselves as advocates for each applicant and are sincerely interested in helping you get into the program of your choice.   However, we cannot   help you if you do not make an effort to help yourself.   Please read this material carefully and make a note of the deadlines.

If you want us to write a Committee Letter:

By MAY 1, 2006 you must:

1.   Open your recommendation file with Interfolio (www.interfolio.com ) if you plan on applying to allopathic or osteopathic medical school.   Please consult a Health Professions Adviser if you are considering other health professions.

2.   Turn in your completed health professions questionnaire and Interfolio document information sheet if applicable (ID code and waiver) to Amy Preziosi in Lattimore 312.   This information may also be submitted via fax to (585) 275-2190 or emailed to Jennifer Sherwood (jsherwood@mail.rochester.edu) or Kate Sgabellone (ksgabell@mail.rochester.edu).  Letters are written in priority order, so it is to your advantage to submit the questionnaire early.

3.   Schedule an appointment with Jennifer Sherwood or Kate Sgabellone for your mandatory questionnaire interview.   Letters will not be written for students who have not had an interview with a health professions adviser.    

WE CANNOT ACCEPT REQUESTS FOR LETTERS WHEN THE APPLICATION DEADLINE IS LESS THAN TWO WEEKS.   

NO QUESTIONNAIRES WILL BE ACCEPTED AFTER AUGUST 2.                                                

PLEASE NOTE: A late fee of $25 will be charged for questionnaires submitter after MAY 1st but before August 2.Checks are the only method of payment, and should be made payable to "University of Rochester." LETTERS WILL NOT BE WRITTEN IF A LATE FEE APPLIES AND IS NOT PAID.

We urge you to obtain at least three letters of recommendation, two of which should come from science instructors. There is no maximum, but we question the value and wisdom of sending more than five or six.   Please remember that it is your responsibility to know what recommendations are in your Interfolio file (if applicable). Check regularly to make sure that your recommendations are arriving.

For those applicant who have requested a Health Commitee letter from the College in the past and are requesting an updated letter, we will accept two forms of updated information. You may email a health professions adviser (email addresses listed above) your updated information. In the email title put "HPQ Update." Alternatively, you may re-submit your original questionnaire with all of the new information in bold. Please label all re-submitted questionnaires with "UPDATE" on the first page. YOU ARE EXPECTED TO ADHERE TO ALL DEADLINES AND PROCEDURES FOR THE HEALTH PROFESSION QUESTIONNAIRE (with the exception of the late fee. Updates are never charged a late fee.).

PLEASE REMEMBER:   SUMMER IS THE IDEAL TIME FOR US TO PREPARE COMMITTEE LETTERS.   THE PICTURE CHANGES DRAMATICALLY WHEN CLASSES RESUME IN SEPTEMBER.   IF YOUR QUESTIONNAIRE WAS SUBMITTED AFTER THE DEADLINE AND YOUR LETTER HAS NOT BEEN WRITTEN BY AUGUST 31, YOU MAY EXPECT A WAIT OF ABOUT TEN TO TWELVE WEEKS BEFORE THE LETTER IS COMPLETED AND MAILED.

            In order to help us write a Committee letter that truly reflects your background (your achievements, interests, strengths, etc.), we need the information requested below.   Please attach as many sheets as you need to answer Sections I-IV.   Please type, if possible.   Keep in mind that the information we are requesting here is very similar to the information you will be providing on professional school applications.   Be sure to keep a copy of your completed questionnaire for future reference!

A. What is your concentration?   Why did you choose it?   What are your academic interests, beyond your concentration?

B. List any academic honors (scholarships, honor societies, etc.).

C. Has/have there been any semester(s) in which your performance did not reflect your ability?   If so, note which semester(s) and explain.

D. Explain any interruptions in your education.   How did you benefit from your time away from the University?

E. Describe all independent study courses, study abroad experiences, internships. What did you learn from these experiences?                           

 

III. EMPLOYMENT--Please include what you learned from and/or contributed to each position.

A.  Please list chronologically, beginning with freshman year, the positions you have held during the academic year and vacations, and indicate how many hours a week you worked.

B. What do you plan to do this summer?

C. Have you financed all or part of your education?   If so, please note if this has influenced your choice of jobs?

IV.  EXTRACURRICULAR AND COMMUNITY ACTIVITIES--Please include what you learned from/contributed to each activity.   Please list chronologically and indicate how many hours a week you participated.

B.  Hospital and/or clinical experience (when, where, what you did/learned).    

C.  Off-campus activities (when, with what group, how you participated).

E.   What are your hobbies and special interests?                     

 

V.  PLANS FOR CAREER        

A.  When and why did you become interested in your chosen career?   What is your first-hand knowledge of it? (e.g., have you spoken with any professionals in this field?)              

B.  What else do you think we should know about you?

 

VI.  ESSAY            

            This part of the questionnaire is important for two reasons: (1) It is your summary statement of the professional and personal goals that you feel are important for us to know.   (2) A similar essay is required by health professions schools, so this is a good chance to start organizing your thoughts.

            What would be most useful for us is a concise and honest statement that describes your motivation for your chosen profession, outlines the skills you have acquired through courses and non-academic experiences, and evaluates your strengths in both academic and interpersonal areas.   We are trying to understand what makes you a unique applicant, in order to incorporate this into your Committee letter.   Please make your essay as specific and personal as possible.
