The University sign-off form has been revised (3/24/08) in response to the mandatory NIH Public Access Policy.  An additional question has been added to page 1 of the sign-off form as follows:

13.         For NIH proposals, do all investigators agree to comply with the NIH Public Access Policy? Please see the NIH Policy for details.

The revised form is available on the ORPA website at http://www.rochester.edu/ORPA/Forms/ .  Please start using this form immediately and discard any other versions you may have on hand.
