Office of Research and Project AdministrationNIH has released the revised PHS 416-9 Individual Fellowship Progress Report for Continuation Support. The revised PHS 416-9 will be accepted immediately for all progress reports and is required for all annual progress reports due on/after January 1, 2010. The PHS 416-9 Individual Fellowship Progress Report for Continuation Support (Rev. 06/09) has been revised to include a new item on Form Page 2, 15.D., for fellows to note if proposed research involving human embryonic stem cells (hESCs) is different from that proposed in the previous submission, including use of a different cell line(s). EFFECTIVE DATE: The revised PHS 416-9 Individual Fellowship Progress Report for Continuation Support (Form Page 2) is now available and will be accepted immediately for all progress reports and is required for all annual progress reports due on/after January 1, 2010. Notable Change to Form PHS 416-9:
Check "No Change" on Form Page 2 if activities involving hESCs are not different from the previous submission. Check "Change" on Form Page 2 if proposed research involving hESCs is different from that proposed in the previous submission, including use of a different cell line(s). If different cell lines are to be used, fellows are instructed to include an explanation of how research plans differ, and provide a new or revised section in Item 16.C (Research Training Plans) reflecting the changes, including the cell line number(s). Only cell lines listed on the NIH hESC Registry as approved for use in NIH-funded research may be used. The PHS 416-9 Instructions have been revised to implement the policy change and are now available at http://grants.nih.gov/grants/funding/416-9/phs416-9.htm.
