Many of you are familiar with the Federal Demonstration Partnership (FDP) Model Subagreement. These forms have been developed by research institutions and major Federal funding agencies in an effort to reduce administrative burden at both ends of the award process. The forms were updated as of 7/2009 and will be used going forward for most subawards issued by ORPA. ORPA recently gave a presentation on the new forms at RARA and will speak of them again at River Rats in November.
Attached are the FDP Contact Information sheets from the Model Subagreement, Attachments 3A and 3B. These sheets will be used for ALL subawards issued by ORPA and must be completed by the department administrator. Attachment 3A is the
Accurate completion of Form 3B is especially important under American Recovery and Reinvestment Act funds when such information as subrecipient’s EIN number, DUNS number, Congressional District and Registration in CCR must be provided. Accurate completion of all fields, particularly email addresses for all subrecipients, will assist your ORPA representative in issuing the subawards electronically -- a great tree and time saver.
