Socio-economic and racial health disparities in birth outcomes continue to persist in Monroe County. In the city of Rochester, these disparities are particularly pronounced in specific zip codes, namely a sector often referred to as the "Crescent." Birth outcomes are associated with particular health conditions that need to be addressed if disparities are to be ameliorated and eventually eliminated. To this end, the Perinatal Network of Monroe County has been involved in community-wide efforts to address these issues in the target areas.
