This password selection form is a tool to help you create a secure Sybase server password. Please follow the instructions and return this form with your End User Reporting System Access Request form.


To designate your Sybase server password, just follow these three easy steps:


Your password will be the combination of your selections 1-3 above. For example:


  1. You selected "V".
  2. You selected "#".
  3. You selected "umbrella".

In this example, the resultant password would be "V#umbrella". You should designate your own custom password using the above technique. PLEASE REMEMBER IT! Once you have been given access to the Sybase server, this is the password that you will use.

If you are using the server via software such as Impromptu or Access, this is the password that you must enter in order to obtain access to the server.
