University/Medical Center Mainframe Account Password Reset (ITSF-007)

This form must be filled out by the owner of the userid in question or their Administrator. EMAIL will be sent to DCS OPS Monitoring once this form is submitted. Once ITS/DCS Operations resets your password they will reply as you directed below.

You should be contacted ASAP in reference to the completion of this request. Further inquiries and additional information pertaining to this request should be made to the IS/DCS Operations at 275-9194 or 275-9195.

NOTE: Your password will be reset and must be changed when you first sign on. An e-mailed confirmation will be sent with your new access information.

Standard password attributes are:

Six(6) to eight(8) characters
The password will expire every 60 days
Inactive users for more than 120 days are revoked
Three(3) password input attempts before your userid is revoked.
