05-06 Undergraduate Dependent Verification Statement for First-Year and Transfer Students - submit this form to our office with signed copies of parent and student federal income tax returns and W-2's by June 1.

05-06 Returning Undergraduate Application - If you are a returning undergraduate student please submit this form to our office with signed copies of parent and student federal income tax returns and W-2's.

05-06 Graduate Student Verification Statement - If you are a graduate student please submit this form to our office with signed copies of student and spouse federal income tax returns and W-2's.

2005-2006 CSS PROFILE Non-Custodial Parent Statement - In addition to the CSS PROFILE, students who come from a divorced or separated household are required to have their non-custodial parent complete and submit the Non-Custodial Parent Statement to the Financial Aid Office. This form can also be securely completed and submitted online at www.collegeboard.com.

2005-2006 CSS PROFILE Business/Farm Supplement Form - In addition to the CSS PROFILE, all students or parents of students who currently own a business or farm are required to complete and submit a Business/Farm Supplement for each business or farm they currently own to the Financial Aid Office. This form can also be completed and submitted online at www.collegeboard.com.

2005/2006 Federal Direct PLUS Loan Application - Please print, fill out, and submit this form via fax or mail in order to apply for a Federal Direct PLUS Loan for the 2005/2006 academic year. We are unable to process the 2005/2006 PLUS Loan application until July 1, 2005, therefore this form will not be available until then.

2005/2006 Financial Aid Request Form - If you would like to take action on any of the loans you have been offered you may print this form and submit it to our office with your request. These actions may include decreasing a loan, declining a loan, reducing Federal Work Study Eligibility, ,and requesting a Federal Direct Unsubsidized Loan if your parents have been officially denied the PLUS Loan. Graduate students may also correct the credit hours being taken in a particular semester/quarter in the event they were misreported on the Graduate Student Verification Statement.
