Final course evaluation questionnaires

Note: The evaluation questionnaire designed by the instructor for each specific implementation of the methods course followed the same format and spirit; however, the specific items students were asked to respond to changed somewhat each time (and also differed for elementary vs. secondary math pre-service teachers) so as to match the activities each group of students had experienced. Therefore, we have chosen include here the text of all these evaluation questionnaires. The readers interested may want to compare the items included in each questionnaire with the agenda of these experiences made available in Section C.
