These documents provide illustrations of the instruments we created to receive feedback from the participants after key professional development experiences, and to structure school facilitator's recording of the growth observed in program participants. Choose from the following list the document(s) you may be interested in reviewing:

Open-ended evaluation questionnaire distributed at the end of the Summer Institute -- this document is a good example of an evaluation questionnaire we designed to gather participants' feedback that would help us improve later implementation of the same professional development initiative.
Evaluation questionnaires asking for a quantitative evaluation of a professional development experience (Summer Institute / First follow-up meeting)-- these documents illustrate a different kind of evaluation questionnaire we designed and used to gather data about participants' satisfaction that could be more easily quantified and reported to our funding agency.
Template for school facilitator's final report on individual participants' progress in the program -- this template was used to organize school facilitator's reports and make them comparable.
