To save time before coming to your appointment at the Counseling Center, you can print and fill out requested forms. Please complete the forms and bring them with you to your scheduled appointment.
Prior to the first visit or the first visit of the academic year the Information Sheet and Informed Consent Form should be filled out and brought with you to your appointment.
All forms can be printed in either Word Document or Acrobat Reader format.
