ENTERING STUDENT HEALTH REQUIREMENTS

FULL-TIME STUDENTS – Undergraduate, Graduate, & Visiting Students

Includes students on the River Campus, Medical Center (MS & PhD only), and Eastman School of Music.

  1. Health History Form & Immunization Report – Submit by June 30.
    • The original Health History Form must be completed and returned to the University Health Service (UHS). A copy or fax of the form will not be accepted.
    • The Health History Form is mailed to students from their Admissions Office after they have accepted admission to the University.
    • Immunization Requirements for Full-time Students – For your reference. This document is in the packet you receive from your Admissions Office.
    • If you do not receive the Health History Form within 3 weeks of accepting your offer of admission, write to hhf@uhs.rochester.edu to request the form. Please include your mailing address.
    • Health History Form and Immunizations Frequently Asked Questions for answers to the most commonly asked questions.
  2. Online Insurance Process – Complete by September 15. All full-time students must complete the Online Insurance Process to enroll in the University-sponsored health insurance or to waive the insurance if their insurance plan meets University insurance criteria. The Online Insurance Process will close at 11:59 p.m. on Friday, September 15. Completing this requirement prior to July 6 for undergraduate students and July 31 is recommended. See Health Insurance for Full-time Students for information about health insurance.

MEDICAL & NURSING STUDENTS

Includes visiting medical students at the University for 8 weeks or more

PART-TIME STUDENTS

Includes students taking at least 6 credit hours per semester or 4 credit hours per quarter

The University Health Service requires incoming part-time students to complete the Immunization Compliance Report prior to the start of classes. This report is used to document your health history and required immunizations.

    Part-time students are not eligible to participate in the Student Health Plan and, therefore, do not complete the Online Insurance Process.
