Q: Can I turn in my form late? I can't get an         appointment with my doctor until after the deadline.
Answer: Our goal is to receive the majority of forms at least 30 days before classes begin; however, we realize some students will be unable to do this for a variety of reasons. You do not need to use overnight mail service unless you think your form will not reach us by regular mail by the time classes start. Remember that because the form is machine-readable, it may not be faxed. If we do not receive your form by the start of classes, you will be assessed a late fee.
Q: Have you received my health history form?         Is anything missing?
Answer: Due to the large number of forms received daily during the summer, we are unable to respond to requests to check for receipt of a form. We will notify students whose forms are incomplete or have not been received. Until that time, you can assume we received your form if you mailed it in the envelope provided or in your own envelope with the same address.
Q: I lost my health history form. Can I print a new         form from your website and use that?
Answer: No, you cannot print a form from the web site. Since the form is a scannable form, we can only accept the printed form that we mailed you. If you have misplaced your form, contact the Health History Form Office by e-mail hhf@uhs.rochester.edu or phone (585)275-0697 to have another mailed to you.
Q: I started filling my form in with pen. Is that ok?
Answer: Yes, that is ok. You can finish the form in pen or switch to pencil to complete the form. You do not need to request a new form.
Q: I don't have a student ID number.         What should I do?
Answer: You can leave the number blank. You will be assigned a student ID number when you come to campus.
Q: I can't get my immunizations completed at home.         What should I do?
Answer: You can call University Health Service at (585) 275-2662 to schedule an appointment for immunizations. Please inform the receptionist that you would like to schedule an appointment to complete your immunization requirement. There is a charge for both the office visit and for the immunizations since completing your immunizations is a pre-enrollment requirement. We will charge the cost to your tuition bill or you may pay by check or credit card.
Q: Does my doctor have to sign my health history form?
Answer:We must have official documentation of your immunizations; however, this does not always mean that your doctor must sign the form. If you have official immunization documentation from your school or from your healthcare provider's office, we can accept that in place of a physician's signature on the health history form; however, we ask that you fill in the bubbles on the form before mailing it.
Q: Do I need to have a complete physical examination         in order to submit my Health History Form?
Answer:Your doctor may want you to have a physical examination; however it is not a requirement unless you are beginning a health profession program (e.g., nursing, medical school).
Q: What immunizations are required?
Answer:Measles (two immunizations, the first on or after your first birthday - no exceptions), Mumps (one immunization), Rubella (one immunization), and Tetanus/Diphtheria (Td, one immunization) are required. The Tuberculin Skin Test, also referred to as the PPD, is required for students from specific countries. (See List of Countries.) Students who have been residing in the United States for the past five years are not required to have a PPD test done unless going into a health professions program. Students who have had BCG must also have the Tuberculin Skin Test if it is required for their country.
Q: Are the Meningococcal vaccine and the         Hepatitis B vaccine required?
Answer:No, these vaccines are not required. The Hepatitis B vaccine is recommended for all students. The meningitis vaccine is recommended for all freshmen. We encourage you to consider receiving these vaccines. You may want to discuss them with your doctor.
