University High Deductible Plan
for Students with Children

The University Health Service offers the University High Deductible Plan for students with a dependent child or children. Students without children are not eligible to enroll in this plan. The plan provides coverage from September 1 through August 31. The cost is $990.79 per month.

2008 Fees for University High Deductible Plan

University High Deductible Plan $990.79/month

Mandatory Health Fee * $48.00/month

* The mandatory health fee will be $50/month beginning 9/1/08.
This fee covers the student only. The fee will be reduced if the student has a primary care provider at UHS.

Students enrolling in this plan must enroll during specific open enrollment periods: (1) when they are first eligible, or (2) when a qualifying even occurs. Once enrolled, students are enrolled in the University High Deductible Plan through August 31 unless they have a qualifying reason to disenroll. Before enrolling in this insurance, students may want to check if they are eligible for insurance through New York State's Child Health Plus or Family Health Plus.

Completing the Health Insurance Selection Process

Health insurance is required for all full-time students. Students select their insurance option by completing the on-line Health Insurance Selection Process every year.

If you are selecting the University High Deductible Plan, you must also complete the Enrollment Form for the University High Deductible Plan and send the form to the UHS Insurance Advisor by fax, mail, or in person.

Suggested Dates for Completing the Health
Insurance Selection Process

Students should complete the Health Insurance Selection Process prior to the start of classes. For undergraduate students, completing the process by June 30 will assure correct billing on the first tuition bill the student receives. For graduate students and medical students, completing the form by August 31 is recommended. Incoming first year medical students should complete the selection process by July 31. (The recommended date for students beginning in May is April 15. For students beginning in January, the recommended date for completing the selection process is December 15.)
