{"id":16956,"date":"2020-01-16T10:03:24","date_gmt":"2020-01-16T15:03:24","guid":{"rendered":"https:\/\/www.rochester.edu\/human-resources\/?page_id=16956"},"modified":"2026-03-05T11:51:08","modified_gmt":"2026-03-05T16:51:08","slug":"family-medical-leave-fmla","status":"publish","type":"page","link":"https:\/\/www.rochester.edu\/human-resources\/benefits\/leave-disability\/family-medical-leave-fmla\/","title":{"rendered":"Family Medical Leave (FMLA)"},"content":{"rendered":"<div class=\"large-4 columns\">\n<p>Family Medical Leave (FMLA) is an unpaid leave granted to eligible employees for up to 12 weeks during a 12-month period for a qualifying reason, including:<\/p>\n<ul>\n<li>The birth or care of a newborn child<\/li>\n<li>Placement of a child for adoption or foster care and care for the newly placed child<\/li>\n<li>To care for a spouse, domestic partner, child, or parent with a serious health condition<\/li>\n<li>The employee\u2019s own serious health condition<\/li>\n<li>To care for a service\/military family member recovering from a serious injury sustained in the line of duty<\/li>\n<li>A service\/military family member is, or will be, deployed to a foreign country<\/li>\n<\/ul>\n<p>You can reference the FMLA policy on the University&#8217;s policies website for full details.<\/p>\n<p><a class=\"u-link-external\" href=\"https:\/\/www.rochester.edu\/policies\/policy\/family-medical-leave\/\" rel=\"noopener noreferrer\">View full policy<\/a><\/p>\n<h2>Requesting Family Medical Leave<\/h2>\n<p>To request the FMLA, please review the following certifications below. Please print and submit the completed medical certification form to the University of Rochester\u2019s Leave Administration Department within 15 business days from completion. Please submit via email to <a href=\"mailto:HR_FMLA@ur.rochester.edu\">HR_FMLA@ur.rochester.edu<\/a> or fax to (585) 276-1361.<\/p>\n<h3>Medical certification forms<\/h3>\n<p>Please click on the appropriate certification for your FMLA leave need and print. When FMLA time is needed, follow department\/unit call off procedures, be sure to indicate that the absence is FMLA. Use of FMLA time is confidential and notice that call off is FMLA should be to supervisor\/designee only.<\/p>\n<h2>Automated process for accruals in myURHR<\/h2>\n<p>Effective December 15, 2024, in myURHR UKG, there will be a new automated accruals payments process that will determine the order in which accruals for time will be paid.<\/p>\n<p>Accruals for paid time to supplement FMLA will be
