Transportation Plan

Please provide your transportation plan for the conference. If you are planning on taking the train or flying to the conference, we will send your information to a travel agent, who will then contact you to book your train or plane ticket. If you are planning on driving/carpooling to the conference, you will be asked to indicate whether or not you will be the driver and the names of the other attendees in the vehicle. If you are the driver, you will be asked to submit a W-9 form and a map with mileage for reimbursement. You will also be asked your parking needs for the conference.
