Fill Out The Online Reporting Form
The form, available at the link below, allows for anonymous, direct reporting, witness, and bystander reports. The Title IX Coordinator receives reports submitted through this form. A report of conduct prohibited by Title IX is not the same as a formal complaint. A report, independently, does not automatically trigger a grievance procedure or investigation.
The University has a designated Title IX Coordinator and Deputy Title IX Coordinators who can answer questions and assist you with reporting options and resources. You are welcome to contact any of the coordinators, even if the person is in a different school or unit than you.
Please note that Title IX Coordinators and Deputy Coordinators are private but not confidential resources; they serve in a neutral role and support all members of the University. They will address your complaint with sensitivity and keep your information as private as possible.
