The secure form, available at the link below, allows for anonymous, direct reporting, witness, and bystander reports. The Title IX Coordinator receives reports submitted through this form. A report of conduct prohibited by Title IX is not the same as a formal complaint. A report, independently, does not automatically trigger a grievance procedure or investigation. This form is not to be used in the event of an emergency.
Title IX Staff
Title IX Coordinators and Deputy Coordinators are private but not confidential resources; they serve in a neutral role and support all members of the University. They will address your complaint with sensitivity and keep your information as private as possible, while taking steps to address your concern. You are welcome to contact any of the coordinators, even if the person works in a different school or unit than you.
