Requirements for Visiting Students: Health Insurance & Immunizations
All full-time registered visiting students on campus for eight weeks or longer must review, complete, and submit the following:
- Health History Form—This is included in your pre-arrival packet from University Health Service. The form contains an immunization record section, which needs a health care practitioner’s signature for completion.
- Immunization Requirements and Forms
Visiting students must either enroll in the University-sponsored health insurance plan or show evidence of other comparable insurance.
NOTE: International-based plans often do not meet University Insurance Criteria, so review these criteria carefully before deciding to purchase insurance from an international-based company. Your plan must meet all of the University Insurance Criteria to qualify for waiver of the University-sponsored insurance.
Insurance coverage date requirements are from the first day of the month in which you begin your placement through the last day of the month in which you end your visit.
Before your arrival, you will receive a University Health History Form with your I-20 or DS-2019. Review this carefully as you must have it completed, including sections filled out by a physician.
You may fax or send a copy of the document via secure email, but the original must be submitted in hard copy upon your arrival at the University. For questions on the Health History Form, email hhf@uhs.rochester.edu.

Visiting students are subject to the mandatory health fee and health insurance (if applicable) to view the current rates, visit UHS’s student health insurance page.
