Instructors should include a statement in the course syllabus inviting any students with disabilities to come and see you about their required accommodations. The following statement can be personalized or adapted to your specific requirements:
Accommodation for a Disability

Recommended Addendum re: Accommodated Test Proctoring
Instructors on River Campus (SAS, Hajim, Simon) and ESM should choose ONE of the following to add to the above statement:
Option One
For students with Office of Disability (ODR) approved testing accommodations, this instructor has made arrangements with ODR for their proctoring assistance for this class. Students must make requests for accommodated exams directly with ODR following the deadlines listed in ODR Testing Policies.
IMPORTANT: If a student requests an accommodated exam late AFTER the ODR deadlines, regardless of the reason, the late request will be denied by ODR for that exam and the student will, by default, take that exam at class time without accommodation.

Option Two
This instructor will make their own arrangements to proctor any students with Office of Disability (ODR) approved testing accommodations. Please contact instructor directly for details about specific testing arrangements.
