We want to hear from you! If you have a concern that you’d like to share with the UHS Leadership Team, or would like to recognize a provider or staff member for their outstanding work, please reach out!
We offer multiple ways for you to share your feedback with us:
- Comment Box – For quick notes of praise or concern, leave us a note in our comment box found in the vestibule of the UHS building. This box is checked weekly and notes are shared with the UHS Leadership Team.
- Patient Advocate – If you have a concern and don’t feel comfortable sharing it with your health care provider, the Patient Advocate may be able to help. Please call 585-275-2679 or email to contact the UHS Patient Advocate, Michelle Livingston. (Please note, this is ONLY for patients of University Health Service, NOT Strong Memorial Hospital or URMC.)
- Feedback Form – Please use the below feedback form to share your thoughts with us. Entries to this form are immediately emailed to a member of the UHS Leadership Team and will be addressed accordingly, depending on the details of the submission.
