Health Requirements for Entering Students
The University Health Service (UHS) has two requirements for all entering students to complete prior to the start of classes:
- Health History Form
- Online Health Insurance Process

FULL-TIME STUDENTS
(Health Profession Students: See below.)
Includes all full-time entering students on the River Campus, in the Medical Center (MS & PhD only), and at the Eastman School of Music:
- Undergraduate students
- Graduate students
- Visiting/exchange students
Does not include:
- Medical, nursing, and other health profession students
- Part-time students

PART-TIME STUDENTS
(Health Profession Students: See below.)
Includes:
- Part-time students taking at least 6 credit hours per semester or 4 credit hours per quarter.
MEDICAL, NURSING, & OTHER HEALTH PROFESSION STUDENTS
Includes all entering full-time and part-time:
- Medical students in the School of Medicine and Dentistry
- Full-time and part-time nursing students in the School of Nursing
- Visiting medical students who will be at the University for 8 weeks or more
- Residents/students at the Eastman Institute for Oral Health (EIOH)
- Psychology Interns
