UNIVERSITY HEALTH SERVICE REQUIREMENTS

Welcome to the University of Rochester! The University Health Service requires incoming part-time students to complete the Health History and Immunization Form and the Allergy and Medication Form prior to the start of classes via UHSConnect. Students will need to register for UHSConnect to complete the required forms.

  • New York State Law and the University of Rochester mandate completion of this form and all requirements. Required tests and immunizations must be completed and verified with a signature by a health practitioner or public health official and uploaded with the form.
  • All immunization information must be submitted in English
  • Students not in compliance with all requirements by the first day of classes will be subject to a late fee and subject to additional administrative action.

*If you are a Part-Time student taking 6 or more credit hours, you will be charged a $35 Student Health Processing Fee,  which covers the administrative cost of processing health history forms and ensuring immunization compliance. There are no exclusions for University of Rochester employees. *

Online Health Insurance Process

Part-time students are not eligible to participate in the Student Health Plan, and therefore do not complete the Online Health Insurance Process.

 

FAQs

Q: What immunizations are required? Students must meet the New York State and University of Rochester immunization requirements. The mandatory immunizations for part time students are:

MMR (measles, mumps, & rubella): 2 doses of live measles vaccine, 1 dose of mumps vaccine and 1 does of rubella vaccine. MMR (measles, mumps & rubella) vaccine may be used to satisfy a single dose of measles, mumps and rubella. Doses must be on or after the first birthday and at least 30 days apart.

Covid-19: Students must receive a Covid vaccine—both doses of a two-dose vaccine or the single dose of a one-dose vaccine—approved for use in the U.S. or accepted by the CDC.

Q: Lab report for Titers? If you have titers (blood drawn) for Measles, Mumps and Rubella (MMR), we must have the actual lab report that says you have a positive reading. Please include the lab report with your form when turning it in. Students with a negative titer are required to receive the immunization(s) for that disease.

Q: Where can I get my immunization records? We must have official documentation of your immunizations. If you have official immunization documentation from a past school, job, or from your healthcare provider’s office, we can accept that documentation.

Q: Can I turn in my form after the deadline? Our goal is to receive the majority of forms from incoming students prior to the start of classes. New York State law mandates that students not in compliance with the requirements 30-45 days after the start of classes are to be withdrawn from school and required to leave campus.

Q: I am a current UR employee. What do I need to do? Do you already have my immunization records? Even though you are a current UR employee, being an employee and a student are looked at separately in the eyes of New York State laws for immunization verification. Occupational Environmental Medicine (OEM) deals with employee medical records while UHS (University Health Service) deals with records for students. Due to HIPPA regulations, OEM and UHS are not allowed to cross reference medical information. The employee/student must do this process themselves. You can call OEM at 585-487-1000 to request your immunization information. You will need to obtain your immunization information from OEM. Once you have your immunization records from OEM, you need to fill out the Part Time Health History and Immunization Form and attach the OEM paperwork as documentation of your immunization history.

Q: If I cannot complete my immunizations before I come to school, what should I do? You can receive your immunizations at UHS after you come to campus. Call 585-275-2662 to schedule an appointment. Please let the receptionist know you would like to complete your immunization requirements. It will be billed to your insurance plan. You will be responsible for payment if it is not covered by your insurance plan. You may be subject to Health History Form late fees.

Q: How do I know if you received my Health History and Immunization Form or if anything is missing? We will notify students whose forms are incomplete or have not been received within 30 days of the deadline. Please always check your spam email.
