Medical, Nursing, and Other Health Profession Students

These health requirements apply to all entering full-time and part-time:

  • Medical students in the School of Medicine and Dentistry
  • Students in the School of Nursing
  • Visiting medical students who will be at the University for 8 weeks or more
  • Residents/students in the Eastman Institute for Oral Health (EIOH)
  • Psych Interns

Welcome to the University of Rochester! The University Health Service requires incoming students to complete the Health History and Immunization Form and the Allergy and Medication Form prior to the start of classes via UHSConnect. Students will need to register for UHSConnect, our online patient portal, to complete the required forms.

Click here for directions on how to register for UHSConnect to complete your forms. Please complete and submit the forms to the University Health Service (UHS) via UHSConnect.

  • New York State Law and the University of Rochester mandate completion of these forms and all requirements. Required tests and immunizations must be completed and verified by a health practitioner or public health official as specified.
  • All immunization information must be submitted in English.
  • Students not in compliance with all requirements by the deadline specified by your program will be subject to a late fee and may be subject to additional administrative action.

UNIVERSITY HEALTH SERVICE REQUIREMENTS

Health History and Immunization Requirement – Must be submitted prior to the start of classes. Please read the following documents for more information.

  3. Services for Full-Time Students
  4. Notice of Privacy Practices
  5. Physical Examination Form

Influenza (Flu) and COVID-19 vaccinations are not required but are strongly encouraged to keep our patients, colleagues, and community safe. Documentation of your vaccination status (whether you got vaccinated or declined ) is required by providing proof of vaccination or declining.

*If you are a Part-Time Health Profession student, you will be charged a $50 Student Health Processing Fee,  which covers the administrative cost of processing health history forms and ensuring immunization compliance. There are no exclusions for University of Rochester employees.*

Online Health Insurance Process

  • All full-time students must complete the Online Health Insurance Process to enroll in the University-sponsored health insurance or to waive the insurance if their insurance plan meets University insurance criteria.
