University Health Service

Health Requirements for Entering Students

The University Health Service (UHS) has two requirements for all entering students to complete prior to the start of classes:

  1. Health History Form
  2. Online Insurance Process

FULL-TIME STUDENTS

(Health Profession Students: See below.)

Includes all full-time entering students on the River Campus, in the Medical Center (MS & PhD only), and at the Eastman School of Music:

  • Undergraduate students
  • Graduate students
  • Visiting/exchange students

Does not include:

  • Medical, nursing, and other health profession students
  • Part-time students


PART-TIME STUDENTS

(Health Profession Students: See below.)

Includes:

  • Part-time students taking at least 6 credit hours per semester or 4 credit hours per quarter.
  • Part-time School of Nursing students in MS-CNL, MS-HCM, MS-MNE, and DNP programs.

 

MEDICAL, NURSING, & OTHER HEALTH PROFESSION STUDENTS

Includes all entering full-time and part-time:

  • Medical students in the School of Medicine and Dentistry
  • Full-time nursing students in the School of Nursing and part-time students in MS-Nurse Practitioner programs (except MD-FMH). Please note: Part-time students in the MS-CNL, MS-HCM, MS-MNE, and DNP programs should use the form for part-time students.
  • Visiting medical students who will be at the University for 8 weeks or more
  • Residents/students at the Eastman Institute for Oral Health (EIOH)
  • Psychology Interns
